I got a call today from a good friend of mine who has noticed a change in her breast during a self-exam.  She has been lactating for seven years now, and her youngest is almost four years old, and still nurses 3-4 times a day.  When she lifts her arms, she notices dimpling on one breast, just at the edge of the areola.  She calls this side her son's "favorite" and does say that in the beginning (and during teething) she had some trauma until they corrected his latch. She does have some discomfort on that side while he's nursing, but thinks that may be cyclical.  She's very nervous and has made an appointment to see her gyn, expecting a referral to a breast surgeon after that.  I'm looking for something in print that I can send her that discusses the differences in screening a lactating breast vs a non-lactating breast.  Is it true that ultrasound is a better first step than mammography?
