Hi Tricia,
Years ago there was a cluster of low weight gaining babies in my own
circle, and the three mother-baby dyads were located in three different
towns and cities across Australia. All were breastfeeding on the baby's
cues, no withholding of feeds; indeed, two were breastfeeding
counsellors. The one who was particularly thin (like a baby in famine) had
numerous tests by his paediatrician (at this distance in time, I'm not sure
what), and nothing untoward was found. Unusually for the time, his
paediatrician didn't push the idea of weaning. Another of the mothers was
reassured by her experienced, older GP. The babies had good medical care.

All three babies started gaining at 5-7 months. The one who was
particularly waif-like ended up being almost 2 metres tall (over 6 ft) as a
young adult and as thin as a rake handle. Another of this cluster of
babies was at uni, the last I heard. There was no explanation, no common
thread, and they lived in very different locations. Two of the mothers were
heavily involved in voluntary work, but they always had their babies with
them; the other was then a stay-at-home mother in a country town.
