Based on the information you have given and on my first impression, I would
say that mom's good supply has dwindled for some reason. The milk transfer,
while just a snapshot of one feed, was low, and baby has taken much larger
feeds (4oz). The other symptoms fit the picture too.
My first thought is the lack of nighttime nursing, and possibly lack of
night time contact with baby. My first inclination is to have mom and baby
co-sleep and nurse during night.
Other reasons could be return to outside employment, contraceptive use,
return of menses.
Can she try to get a few more feeds in per 24 hr day? If possible, during
the night? Is baby very nearby where mom can hear his cues? or is he in
another room? In my experience, it is unusual, but not unheard of, for an 11
wk old to sleep 7 hrs, but I find it does often compromise the milk supply.
