What services do MDs typically bar hospitals from providing to their
patients?  Could an MD write an order against radiology, for instance, if
the patient wished to have a mammogram and the MD did not wish her--or any
of his/her patients--to have one there?

If a physician bars a woman from receiving a mammogram, or from receiving
appropriate lactation support, and her health subsequently suffers as a
result, is the physician liable?

In a situation where a physician of one specialty wishes to bar a physician
from another specialty from providing services to his/her patient, how is
this accomplished?

Can a physician bar independent service providers--for instance, could a
physician write standing orders banning all his/her patients from receiving
visits from all massage therapists in the community while in the hospital?
Could this extend to the patient contacting massage therapists on her own
when out of the hospital?  Would the massage therapists have a legal
obligation to refuse to see any patients of a certain doctor who had written
such an order?  Would the situation be different if the profession were
psychotherapy?  If so, why?

If an LC wished to prevent a certain MD from seeing the mothers she is
seeing, providing she saw the patient first, could she do that?  If not, why
not?
