Many components in human milk are active against e. coli.  I have seen
patents on lactoferrin that specifically mention the use of this component to
inactivate e.coli.  The following website has wonderful tables in regard to human
milk components and the microbes that they inactivate. see

E. coli is killed by heat.  Sterilizing bottles and nipples should
technically eliminate any problem of washing this equipment in contaminated water.  I
cannot see any reason to heat treat human milk or to go to infant formula.
