I just made a home visit to the mom of twins who reported to me that she
found that using the insert when she used the pump, it was much less
painful and she was able to get more milk.

This was surprising to me, as her breasts were large and soft with rather
large pliable nipples.  She is now breastfeeding her twins, but still pumps
at times.  While they were in NICU she was able to provide all the milk
both needed with the electric pump (which, but the way, she used one side
at a time as she preferred it that way even though it is a double electric
pump.) 

When I observed her using the pump, I realized that due to the very pliable
nature of her breast tissue, without the insert her nipple was pulled in
very deeply (and painfully). The insert prevented this problem.

I just bring this up so that those who are helping moms use pumps who
report pain while pumping consider the use of the insert when it may not
seem logical...
