With permission to post:  I am working with a mom with sore nipples...nothing earthshaking but baby is basically rolling bottom lip in and pinching away to get milk.  The real problem was that parents were using "5S's" to comfort baby everytime he cued for feeding and were so proud they could comfort him so completely!  He had had only 1 wet diaper in 24 hours and no stools in 48 at day five!  Oh, yes and there was that nipple shield tossed on the nipple on the delivery table!!! despite wonderful nipples for breastfeeding!

Baby latched relatively easily without shield with a bit of expressed milk to lure him.  Mom nursed 'all' night and pumped 3 times after I did a home visit at 6pm and by the morning MD visit the little guy had peed 8 times and stooled 3!

Now 3 days later mom is still reporting sore nipples although there is minimal nipple damage (damage is overstating) maybe irritation...  The real issue may be that she chose an epidural that didn't work in labor and ended up with a spinal headache and major back pain.  She did finally get a blood patch which helped a bit.  She is only comfortable nursing laying down and she doesn't really seem to be able to get a good latch in that position.  

Any suggestions?  She is wearing down however, I suspect it has more to do with the head and back pain than the sore nipples.  She doesn't have anything that seems to help those so she is focusing on the breastfeeding (after all we all know there is something she can do about that---use those bottles of formula so kindly provided by the hospital).
