Sorry, I don't know the origin of the phrase (nipple sandwich)  but am glad
that it was brought up because although I like the positioning, I have
trouble with use and understanding of the term. When coaching parents I often
will use these words to describe what I am doing but inevitably they develop
a perplexed look on their faces, they just don't  "get" the imagery.
 Does anyone have a more ideal way of describing this process?
I would like to leave the word "sandwich" out entirely as this is where I
seem to lose them!
