One thing that I don't think has been mentioned as a strategy to get more
food into this baby is to wear baby in a sling or carrier.  It frees mom's
hands for her other duties and gives baby free access to the breast.  If she
doesn't have a sling she can easily make one out of a long length of fabric.
It's a great strategy for moms of toddlers with a newborn, but would
certainly apply with this almost 6 month old too.  

And if there are resources for her to get the TT/ULT released, I would
strongly encourage it .  Sounds like her last child may have had the same
problem.  Who knows about the first, since she didn't breastfeed that one.  

We have all seen breasts that seemed like they couldn't possibly produce
enough milk for exclusive breastfeeding and have been totally wrong.  Sounds
like baby is able to transfer an adequate amount but the other challenges
are hindering desired growth.
