Help I am to see a mom for her 1st postpartum Brf visit after 12noon peds visit,

scenario:  25 yo primip with questions re: augmentation surgery X 2 both
above muscle and incision was peri-areolar lower 1/3 segment bilat.
Delivered boy thurs night 1730 and has been latching infant on since
immediately after delivery with feeds lasting 20 to 40 mins.  MOM  stated
yesterday that latch was "good" swallows heard but gets "shooting" pains
(reported to be tolerable) upwards from intact portion of areola (bilat) no
pain underneath distal from incision sites......today, Day 3 of life baby
had 6 wet diapers and 1 stool!  mom is now engorged (mildly) with good
softening of upper glandular areas.. Problem is, no softening occurs after
nursing (varied positions) on the lower lobes.....When we expressed on the
prenatal exam it was obvious the upper nipple openings flowed freely, as we
expressed the lower areolae no colostrum flowed....I plan to recheck this
tomorrow, I also plan to do a test weight at the visit.....

My advice:    since she had two potentially destructive incisions on the
lower peri-areolar areas I asked that she put one cabbage leaf over non
softening lobes at all times while not feeding and periodically (1X every
1-2 hours place frozen diapers over cabbage leaves)  Since I have not had
luck previously with getting these incision sites to flow with other moms,
My intuition is to try to suppress the lower damaged areas...Am I on the
right track? or is it impossible to selectively or discreetly isolate and
suppress glandular function?  My thought is if we can suppress it is better
than trying to re-establish continuity with scarring likely and the
likelyhood of repeated plugged ducts and/or abcesses.....without  being God
how can I tell when I examine her what her chances of normal lactation?????

Has anyone in LACTNETLAND tried this aproach  I have time to change the
course if you have solid ideas....This mom appears to be very motivated and
compliant for any "holistic methods available" She regrets deeply her
earlier decision to augment She has silicone inplants from originally 7
years ago She "wishes I never had it done" How can we keep this from
turning into a "train wreck", I warned her pre-natally that plugging and
infection was my biggest concern and stated she had at least a 30%-50%
chance of lactating at all but a 50-80% chance of having breast problems.

WE are both waiting for any other ideas or enlightenment on this
case.....Thanks in advance for your time....ruthy.

p.s. Dr Newman and/or Dr HALE has motillium gained FDA approval stateside yet?
