I had a woman once who had been diagnosed with a single dermoid ovarian 
cyst prior to getting pregnant - they had decided not to remove it because it 
wasn't causing any major problems, though the plan was to probably remove it 
sometime in the future. She had gotten pregnant uneventfully, but she could 
not make milk, the most she ever got was one quarter ounce combined from 
both sides despite all of our efforts to increase her supply. After about 2 
months she gave up - I have always suspected that cyst was the reason she 
couldn't make milk. I think a blood test for testosterone makes a lot of sense, 
but I also wonder whether it would be possible to ultrasound the ovaries to 
visualize what is going on. If it is a dermoid cyst for instance, which can have 
identifying features like teeth and hair, then that would be a permanent 
situation versus  temporary cysts that might resolve in a month and a half. I 
am neither a doctor nor an ultrasonographer, so I don't know how much one 
can diagnose via ultrasound, or even how easy it is to see the ovaries on a 
woman who has just delivered, but it might help refine the prognosis if one 
could figure out whether there is a  cyst, and if it  is permanent or temporary.
