Hi Kristi,
This mother does not automatically need to supplement her baby, but she and
the baby absolutely do need information on effective feeding, diaper output,
usual weight loss and gains, and early followup. Mother needs to know what
signs indicate that the baby does need supplementation, and if signs are
present, how to immediately get further help.
From your description of the situation, it does not sound like the baby
needs supplementation.
Personally, at 60 hrs old, I would already want a weight check, under these
circumstances. And if mother and baby have been breastfeeding unrestrictedly
since birth, I would expect to see the milk supply noticeably increasing by
this point. The milk supply would not necessarily by "full in" but usually
changes are being seen.
I cannot explain why the mother had been told she would definitely need to
supplement, perhaps the IBCLC has only seen cases in which milk supply was
very low following such surgeries, or perhaps mother somewhat misinterpreted
what she was told. Neither scenario are very far-fetched.
