My first recommendation is that the mom get a second opinion.   Perhaps a
pediatric ENT?  (Ear, nose & tongue??)

In any case, this needs to be checked out.  If it is causing a deviation of
the tongue at 4 weeks, what will it do by the time the child is 12?

It is already interfering with ADL -- activities of daily living if the baby
can't breastfeed.  She should see someone else.
