Just a thought, which you may well have tried, Jennifer.  In 'Bestfeeding'
by Renfrew and Fisher they illustrate the use of a homemade sling to support
the large breast, freeing mother's hands and giving the breast more firmness
without distorting the shape.  Perhaps this could make it easier for your
client to at least manage with just her own two hands.
I can't do justice to the illustration in a verbal description, so advise
getting hold of the book and looking at it there.
