I have  never had any multiples but the way that I plan to handle it is
to charge the basic consultation fee (for 2 hours) and then charge by the
hour (billable in 1/4 hour segments) for any time over 2 1/4 hours.  If
you think that will come out much higher than charging for each baby I
guess you could set a special flat fee.  I can't see myself charging two
or three times the basic consult fee though unless I was really there for
that long.
