Andrea,
I have used a Lact-aid (or SNS) is cases similar to this.  Sounds to me
like this couple need to be doing some serious nursing, and a supplementer
at the breast will help baby not get impatient while he waits for letdown.
It will also stimulate mom's supply. She can used her own expressed milk
for the supplement.  Her diet sounds good.  Is she (or baby) on iron
containing vitamins?  Could be a possible culprit.
