Yes, this is a complaint I hear about fairly frequently.  It is hard
to know how common the problem is because people do not contact me, as
I am sure they do not contact most of you, just to tell you that the
baby is 4 months old and the milk supply has not decreased.

Now, I only include in this group women who have been breastfeeding
exclusively, the baby had been growing very well, at least until
recently, they never give (or only very rarely give) bottles, and are
not on the birth control pill.

The complaints are that the baby is not satisfied, pulls away from the
breast (unlike a "growth spurt" the babies do not nurse longer and
more frequently), sucks his hand rather than nurse, and may even gain
little or no weight over weeks.

I don't know how to explain it.  As I mentioned, a brief course of
domperidone usually brings things back to normal.
