I would ask mom about anxiety symptoms/prior diagnosis. I have seen anxious moms who will do not let down well. Treatment (counseling +/- medication) can help. So can awareness, plus using more physical strimulation.
BEFORE putting pump or by to breast- roll or gently rub nipples for two minutes, OR until letdown occurs, then apply baby or pump. IF no let-down at the end of two minutes, lean forward and shake breasts back and forth a few times, then apply baby or pump. This almost always works, and as mom restores faith in her body, less and less intentional manual stimulation of the nerves is necessary.
(We had one mother who could only let down for her baby in our support ggroup room at MilkWorks for the first few weeks. Her husband and toddler came and lived there during the day while she nursed the newborn off and on and gained confidence/decreased anxiety.)
