This sounds to me like a nursing strike.  I know it's unusual for a strike to last this long. You said she was suctioned at the hospital when she turned blue.  I'm wondering if there was some damage done to her throat at that time that made nursing hurt.  Then she quickly associated pain with being put into a position for nursing.   Hence the fighting and turning away.  My 4 month old, totally breastfed baby went on strike when she got a cold.  Just laying her back in my arms to feed would cause her to arch her back and fight me.  I fed her in her sleep but needed to feed her more than that.  I would lay her across my lap and talk to her for a few minutes.  I already had the flap on my bra down but my shirt covering me.  Suddenly I would lift my shirt and lean in, not picking her up to put into position.  The breast would be right there and she started taking it that way.  The first few times I had to do the whole feeding in that position.  But before long, as soon as she was nursing well and milk had let down I could slowly pick her up and hold her normally.  Just a thought.
