Just to add a comment to the excellent posts from Laurie, Pam, Susan,
Barbara and others about this baby who is doing very poorly indeed:
Definitely calories - and the feeling of a *full stomach*. This little one
has never experienced a full stomach and so continues to be a "happy
starver", not knowing what s/he is missing. Having a really full
stomach from a greater intake at a particular feed (more calories), or feed
plus top-up, also provides the experience, that sensation of fullness, so
that the baby knows what to expect. (Yes, I acknowledge that feeds vary
in volume.)

I just added this thought to support the excellent comments already
offered. Sometimes a baby who has never experienced fullness isn't going to
look for it and will be congtent with less than needed.
