Hello wise ones,

Has anyone had experience helping a mother who has one-sided decreased supply and thinks it is from inhibited let-down?  The mother of a 4 month old is back to work and consistently gets "4-5 ounces from her left side, but is lucky to get 1 ounce from her right though the right feels full".  She is crunched for time at work and is stressed about the uneven pumping amount.  She still feels full on the right side after pumping too, left is soft after pumping.  I encouraged her to simultaneously pump both sides under a cover while looking at baby's picture to help her relax.

When she and her baby are together, they are nursing.  Whether she starts on the right side or the left side, she says the baby always latches and nurses more comfortably and less distractingly on the left side, thus emptying the left more completely, leaving the right side feeling a little softer, but not as soft as she thinks it could be.  What am I missing?  Breasts look normal, no surgery, nipples look normal, latch and positioning look comfortable and effective other than baby not staying on the right side as willingly.  No blocked ducts or fullness seen today and mother states she feels no block currently; that it comes and goes.  What could be causing her right side to not let down or repeatedly have fullness that leads to blockages?

Thanks for your help.

Rita, RN IBCLC
