Heather,
I have a small pp.  I read Linda Smith's book and found it incredibly
helpful.  I think it's also important to ask around to local chiropractors,
ot, and pt in pp to see how they got started, what billing software do they
use, rates etc.  I've found these businesses and practices an invaluable
resource.  Make sure you are priced right and not too low for your market.
 I've learned a lot by asking questions of everyone related to health care
from insurance companies to other private practices.  Small, local shops
geared towards moms are also a fantastic resource for supplies and clients
as well as pediatricians, birthing classes, and obgyns/midwives.  I think
it is worthwhile to outsource book keeping and find a good accountant as
you set up your practice.
