Mom of a 5 day old baby has severely eroded nipple tip and serious crack at the base of her right nipple. The wound appears healthy. Left is not so bad. Engorgement is very bad. Breastfeeding pain is very bad. Baby's mouth appears completely, totally normal. The most normal mouth I've seen in months. Mom works in an ICU in a  hospital. We resolved the engorgement and she's going to take a nipple vacation. Question: because she works in a hospital, and the damage is fairly severe, should she use something other than mere nipple ointment, for example bacitracin or neosporin? how about a saline rinse?
