Placental retention-vag birth, bright red bleeding never changes per usual 
stages for lochia; post surgical retention - slim to none occurance.
Delay of lactation - most common cause -you named it- separation of mother 
and baby, lack of breast stim.
Sheehan syndrome is due to really excessive blood loss, I don't think it is 
very common.
Preterm delivery -lack of stim.  At Children's Hospital of Philadelphia PA, 
deliveries are because of something very wrong with baby - will need fairly 
immediate surgical repair - pumping begins within 2 hours after vag delivery 
and within 4 hours after c sec.  Pump early and often!

I was afraid that was what was happening in the cities, intervention where 
unnecessary and no intervention when needed in the countryside :-(  Sad 
commentary on the medical profession.  Pat in SNJ
