Thank you, Nancy and others for the information on MRSA. I have permission
to post and wanted to ask more about this client's specific situation. The
mother had a place on her arm and mastitis in her breast that both had a
positive MRSA culture. She is now taking Bactrim (she is allergic to a lot
of antibiotics, but tolerates this one) and is breastfeeding her 3.5
week-old baby. The hospital physician told her that it was okay to continue
to breastfeed, that it was up to her. So she did so over the weekend. Then
on Sunday the baby had stool that she described as very runny, a large
amount, and dark brown. The baby also had a significant amount of formula
in the days before this was diagnosed, as the mother was going back and
forth to the hospital and leaving the baby with family. But then over the
weekend she was back to only breastfeeding.

The infant's physician told her that she could not breastfeed while taking
bactrim and that she should be bottle feeding formula until she is off of
the antibiotic. When I looked up Bactrim in Hale, it is an L3 and says to
use caution in neonates with hyperbilirubinemia. The mom said her baby did
have some jaundice in the beginning, but she didn't know much more than
that.

Any advice? She does not have an open wound on her breast. She just had a
hardened, read, painful area that was not getting better, and so they drew
some fluid off to take a culture. The place on her arm is open but she says
she keeps it covered.
Thanks for any suggestions.
