ENT's in our hospital would require the baby to be under general anesthesia, and usually try to dissuade mothers from having the surgery for posterior tongue-tie, as they aren't very familiar with it, and don't seem to understand how much PTT affects breastfeeding.
I prefer to send my patients to see a dentist in the area who does laser surgery for any type of tongue-tie, which does not require general anesthesia, and prevents any excessive bleeding in cases that are more involved.  Catherine probably knows more about this as well...
