Hi Sonya, I will try to recreate what I would write (in a note to
doctor, in the progress notes where I usually chart on the baby, or in
a written report after a consult).

Dr. Smith,
Baby Jane has not been able to latch effectively. Mother reports the
baby either cannot latch, loses her latch easily, and makes clicking
sounds when attempting to suckle. I have worked with mother
extensively, using many different positioning and latching strategies.
Even with skilled help, baby is unable to achieve a sustained latch. I
have noted a thin tight lingual frenulum attached under the tongue and
near the tip. Mother states her goal is to exclusively breastfeed in
the early months and to continue bf for 6-9 months. At present, mother
has to pump and bottle feed to provide any breastmilk and baby has
also required formula supplementation. Would you please evaluate baby
for ankyloglossia and possible release of tight frenulum.

An alternate would be: mother complains of "7" pain (0-10 pain scale)
throughout feeding, and her nipples are compressed after every feed. I
have noted very few swallows when baby is feeding, and baby's weight
gain has been xyz. Something like that
