Well there could be several things going on. The baby might have a swallowing disorder. Are you thinking of laryngomalacia or tracheomalacia?
Maybe that is why baby has not gotten going with bf yet, as he is having difficulty with the suck/swallow/breathe sequence. Of course we know all artificial nipples are different and many do not really do what they are advertised to do. That being said, most term babies can suck/swallow/breathe with a bottle nipple pretty well. It could be that the calma teat is still too fast, it could be the way the baby is being held (not upright enough, neck too flexed, etc) or paced (responding to baby's cues and need for breaks etc).
As far as getting baby to mom's breast, figure out if there is a medical problem, then go to the basics of STS, biological nurturing etc. Great that mom is exclusively breastmilk feeding.
