Hi Tina,
Seems we are having a run of sore nipples not healing even after many
different topical treatments!
For your client, the saliva might be making things worse by
re-introducing germs which keeps the wound from healing.
I would suggest mild soap and warm water cleansing several times per
day, and pat dry with a soft clean cotton cloth. Her hands should be
clean when dealing with the wounds. Since you are seeing 'pinching',
and you said the baby 'manages' mom's very good supply, perhaps more
could be done with positioning (prone) and tweaking a possible
overactive situation. I also feel that analgesic/anti-inflammatory
meds can help (not tylenol) like ibuprofen or naproxen (Advil, Aleve)
around the clock for 2-3 days, while working on this.
