I have a mum who I consulted on Saturday who had a milk blister on (L) nipple and blocked duct in (R) side.  Baby boy 10 weeks old, feeding well, mum small breasted, baby gaining well and healthy.  Stools colour are light green (transitional stool colour - Breastfeeding Atlas) - observation when nappy changed
on arrival mum woke baby & he fed from L side, and fell asleep,  so we hand expressed R side, initially and then managed to wake baby for a further feed from the R side.  baby feeding 2-3 hourly settling well
I managed to clear the blister using a damp cloth, and clear the blockage with hand expressing.   Gave the suggestions as per  Riordans, and usual treatment options for this. 
Mum has just called me again - the blister is back and still breasts feel lumpy.  Mum says it is still quite painful.  No breast damage or surgery in the past. Any suggesstions would be most helpful so I can pass these on.
