I doubt that there are mny of us who practice in EVERY area the exam covers.  I tried to concentrate on those areas (as defined in the info you get when you apply for the test) where iI had the least experience.  For example, working in a hospital OB department, I felt very comfortable with information regarding the newborn period, while I was a little shaky with the older baby/child, especially having no grandchildren at that point and little contact beyond the first few weeks.  Someone primarily functioning as La Leche Leader, on the other hand would likely be more comfortable with a bariety of ge groups, but less with the immediate perinatal period.
With apologies to Linda,I personally didn't find her review book that helpful content-wise as the exam covered a very different mix of questions.  (In fairness, I wsas using the first edition as the 2nd wasn't out until shortly after I was reviewing.)  However, if you haven't taken any similar type exams recently, it was good practice in technique for answering questions.  The study guide for Breastfeeding and Human Lactation had test questions for some of the chapters which were also good review.  Breastfeeding Atlas was extremely helpful in reparing for the "clinical" portion.  I would swear some of the exam pictures came from the book or at least they were very similar.
I have to second the comments others have made about reading too much into the qwuestions.  The more experience we have, the more we want to look beyond the stated question to what may be behind it!  I would say to myself, "Well, if this is what's going on, then answer "a" is right, but if the other is happening, then the right answer would be "b"!"  Be sure to note that such questions frequently say something like "Your first action would be....".  First is the key word.  It's not assking what you would do after a half hour interview, assessment etc.
Good luck to all working toward certification.
