Don't faint but my team of hospital LCs has been asked for input from the OB Nursing Leaders on how to monitor if skin to skin time is being given to mom/baby pairs and if supplements are given only according to policy.  As most of you know, these are chronic problems we deal with in the USA hospital setting. 
Does anybody have a method of tracking these things and charting that has worked well?
