As a hopital based lactation consultant I have been working with this
mother/baby dyad since infant's first feeding after birth.Infant was
born yesterday. Mother stated to me at the onset that she has very
sensititve nipples and has been concerned about the comfort of
breastfeeding, but she is very motivated to do so. Mother has a hx of
breast Augmentation and states that she began having sensitive nipples
after that surgery. I assisted her with latch on after skin to skin
contact provided and infant latched on with a wide gape. From
appearances her latch look pretty good to me, but mom could not tolerate
the pain. I assisted her with relatching in a variety of positions. A
nipple shield takes the edge off the pain, but it its still signifigant.
Pumping with a Lactina breast pump on low suction was also very painful
for this mother. Today infant nursed with nutritive suck for about 15
minutes and fell asleep. Mom grit her teeth and dealt with it. Infant
has received just a small amout (5cc) of formula early this morning, but
I 'm pretty sure that I am not looking at nipple confusion at this
point. Anyone have any insight for this situation. This is the worst
case of nipple pain I have seen and yes, I will continue to evaluate the
latch because, you never know, but I suspect that it is not the issue. I
do have permission to post this, mom is eager for some answers.
