     Consider yeast and/or staph of the nipple and raynauds or
vasospasm.   If  you think infection is the culprit, you could treat
emperically with APNO cream or culture nipples careful not to get any
milk on the swab. Another possibility is that the flange opening is too
small.  Don't assume that because she had this before that it is
related.  Just start from the beginning as you would with any other
mother keeping this tidbit in the back of your milnd in case the usual
