Dear Emily
I am not experienced enough to answer all the questions on your survey but I 
would very much appreciate getting the results..Especially the question 
concerning reasons for recommending pumping in the hospital.  We recommend, 
though I know there are some who will agree and others who will disagree 
with  this, that any mother whose baby does not breastfeed at all or does 
not breastfeed well, or who needs for any reason supplementation, by the 
second day after birth, for any reason, should start pumping, together with 
continued tries at breastfeeding, and supplements of formula until there is 
enough mother's milk to do without formula, or the baby feeds well enough. 
By "start pumping" I mean every three hours, including one at night at 
least.  Our  rational for this is: we don't know when the baby will start 
nursing well - maybe NEXT FEED - and maybe only in another few days or more. 
.  Starting pumping early , even though I emphisize to the mothers , that 
the first pumping sessions are not expected to yield colestrum (sometimes 
they surprisingly do..)  definitely starts the breasts to working, and then
1/ by the third or forth pumping sessions there is usually some mother's 
milk for the baby who up until now had formula  2/ this is very good for the 
future milk supply 3/  this helps prevent engorgement.
I decided to send this letter to the group , even though , and because, I 
can imagine that there are many different opinions about this 
recommendation.  Since I am relatively new in the field of breastfeeding 
during the first days after birth, I asked several IBCLCs in the area where 
I work, about this practice/recommendation, and the general idea was "Make 
sure the mothers keep up their milk supply if the baby /mother doesn't 
succeed at breastfeeding  during the first days after birth, and have the 
babies get as much mother's milk as possible, and then when they go home we 
(the lactationists) will help them breastfeed.".
I would be appreciative of any opinions and experiences that hospital based 
and non-hospital based IBCLCs would be willing to share with me.
