I carry Medela products but have seen my sales of larger flanges dropped
significantly lately because I am trying out a newer product that most moms
LOVE.  They are called Pumpin' Pals angled breast shields.  The do not have
the hard angle that the other flanges have, they gradually flare out
accommodating 22-30mm in one flange.  What I see is that the moms areolar
area is compressed and milk is removed better with much more comfort for
mom.  I believe they are working on a flange that will accommodate larger
than 30mm.  Personally I like them because the angle of the flange (which
fits into other pump companies equipment) is such that moms can sit back and
relax rather than having to sit upright the whole time or risk the milk
leaking out the bottom of the flange.

 

I do agree with others tho - you have to really see it is size it.  Can't
tell you how many times I've seen a small nipple expand out to be too large
for a 24mm flange and one that looked like it would be fine in a 30 but
desperately needed a 36.  The measuring tools can be helpful but your eyes
and moms pain or discomfort level are what you really need to use.  :-)
