This is only a study of two, but with my last 2 babies (unmedicated VBACs) 
who were born in the hospital with a midwife, I had a full milk supply in less 
than 24 hours.  How did this happen?  I did short stays ( 24 hours with my 35 
weeker and 6 hours with my full-term) and during my "short stay" the only 
separation under an hour occured for a bath and vitals.  And even these 
procedures were delayed for several hours after birth.  Of course, we also did 
uniterrupted skin-to-skin!  I even had milk in less than 72 hours with my first, 
who was the birth experience from hell - C/S under general anesthesia after 
4.5 hours of pushing (unmedicated up until we did the C/S) and at least a 6 
hour separation after birth.
