Is the 24 mm the smallest shield she has tried? There is a smaller  
insert that fits into the 24mm. Other ideas:
- The Softfit shields may help; unlubricated, the silicone surface  
tends to adhere somewhat to the breast skin and they may help hold  
things in place more.
- Another thing to try might be lanolin between nipple and shield,  
which is also quite sticky and might reduce the movement. (this is  
the opposite of what most pumping mums want!).
- Or try fitting an Avent soft insert into her 24mm shield.

How does she go with hand expressing?
