This has been an interesting discussion.  While I totally agree that 
birthing practices have a very big impact on breastfeeding (form 
personal experience), I also see the random nature of the problems (also 
personal experience -- my third breastfed child, born naturally in a 
birth center, had my nipples bleeding within 45 minutes after birth).  
What I try to emphasize with moms is that breastfeeding should be a 
pleasurable experience, and if they have concerns/problems/pain, that 
they should call me sooner rather than later -- don't be a martyr and 
suffer needlessly.  Yes, a good birth experience reduces the odds of 
problems, but doesn't eliminate it.
