I like the "bait and switch with a nipple shield" trick for older 
babies. Bottle feed him with a wide base nipple (Gerber comfort latch, 
Playtex NaturaLatch, or the new one that comes with the Whittlestone 
hand pump) right next to the breast, while mom wears a nipple shield. 
She sneaks the bottle out of his mouth and presents the breast, if baby 
cries, she gives the bottle back for a few seconds.

Another thing to try is distract and swoop. Mom lays baby on back on 
bed. SHe gets on her hands and knees, and tickles him with the nipple on 
his belly, chest, ear, cheek, then finally right above his upper lip. He 
should reflexly open up his mouth, and mom can let the breast fall in 
from above as he opens. It helps if she makes silly noises with every 
tickle.

Self attach distraction is also good. Mom lays down on bed or floor 
without a shirt or bra on. She takes some rattly, squeeky interesting 
toys. Lays baby on her belly or chest and shows him one of the toys. 
When he is distracted and enjoying himself she offers the breast. If he 
cries, she takes it away and repeats.

THe other thing she can try is "tough love". Offer baby the breast or a 
cup, and that's it. Most babies will at first choose the cup, but then 
quickly try the breast again. If baby gets very upset, mom cuddles him 
and then returns him to her chest.

BUT before I do any of this, I'd want to know what made babe so adamant 
against breast. Was it flow issues? Difficulty coordinating swallowing 
and breathing? Just unfamiliarity and he's a pretty inflexible babe?
forgive the shorthand, am stressed by a lot of projects, but wanted to 
answer this!
