I have a mother whose nipple tip craters in, almost like a volcano.  The 
nipple itself is erect, but the top surface folds into itself.  Each time 
the baby latches and sucks, the baby pulls the crater out and breaking the 
adhesions and re-injures the site.  It is now bleeding at every feeding.  
This is this mom's 3rd baby.  She is very careful about the latch.  
Hydrogel dressings don not help because after a feeding the crater inverts 
again and the hydrogel doesn't touch that site.  I've suggested she talk 
to her doctor about the possiblity of an antibiotic cream like bactroban.  
Even a pump will pull that crater out and cause bleeding.  Any other 
suggestions?
