Hi All;
I have permission to post. A client that was unsuccessful with firs
sibling is 5 wks pregnant and wants this next baby to be able to
successfully breastfeed. She tells me that she has hyperplastic breasts
with insufficient glandular tissue. She says she never had engorgement
with her last baby.
The baby and she were separated after delivery for routine admission
and he did not nurse when he did come back to mom. Had difficulty
getting him to latch and was supplementing by time of discharge. I don't
know how she has come up with this diagnosis. She is a RN and her sister
is a M.D.
My thinking is that there wasn't enough stimulation at the breast for
her milk to come in well. I told her I would need to research
hyperplastic breasts to give her any other advice other than skin to
skin and no separation until after the first feeding or 2 hrs. Rooming
in with baby and feeding on cue. Can pump a little after feedings to
stimulate milk supply. She already has tingling in her nipples. I told
her that her first pregnancy might set her up better with this one. I
also told her I was going to gather all the information I could and if I
can't help her find someone that can. She already called a lactation
consultant and was told she didn't know what she should do. I don't want
her feeling failure before she has a good chance to correct what might
not have been a problem. I haven't seen her only spoke over the phone
but she tells me that her breasts are not asymmetrical. She wears a 38C
bra.
