This isn't much help to you, but I'm posting by way of illustration that experiences with this disorder can be very different.  I attended a woman as doula and later as an LC last month.  She was diagnosed with Factor V Leiden Disorder (homogenous) during pregnancy and took Heparin during pregnancy and began Coumadin following the birth.  She had a posterior baby, was in labor for 3 days and then had a c-section.  The baby had some initial latch problems, but her milk came in on day 2.5/3 and she has oversupply issues.  Perhaps your client's milk supply down regulated during the engorgement phase?
