Lots of possibilities here, but the most likely is tongue tie.  Infants
change their compensation strategies over time and according to the
demands of handling or obtaining milk flow.  I would bet that this mom
is sore from this baby's sucking, and not from him popping off all the
time. My firstborn was hypotonic and tongue tied and fell off my breast
every few sucks...no problem with pain once we got the latch right.

If you can't figure out what's going on, first try making the latch as
perfect and asymmetrical as you can.  If that does not help, go along
with the mom to a more experienced LC and learn from her while she helps
the mom.  If there are no more experienced IBCLCs in your area, then try
an occupational or speech therapist or an ENT physician.
