I have been a lurker (love Lactnet learn heaps and keeps me sane with daily
frustrations of trying to implement BFHI) and this is the first time I have
posted. I have worked in community well child health for 16 years and have
held position of breastfeeding advisor within maternity services of
regional hospital (approx 2000 births pa) for the past 2 years. I am an RN
and have been an IBCLC for 9 years.
Mother's permission to post - AW, a 26yr primip delivered yesterday -
uneventful delivery of ft infant and so far BF going well. Generally good
diet, no supplements, good health. Her hx - 3 terminations and 2yrs ago
miscarried and developed mastitis 2 weeks later (R) breast rx with ABs but
abscessed within 3 days.  Incised surgically but recurred a week later and
was surgically drained again with continued AB' settled. 3 months later (R)
breast abscessed again, surgery, drainage ABs.  During this pregnancy at 3
months (L) breast abscess -surgically drained and flared a week later,
scanned NAD but further surgery drained 50 ml pus. Continued on clindamycin
but abscess flared again just before 3rd trimester - further surgery with 3
different drainage areas - one area still some pussy discharge.  The AB was
changed to Fucudin but then changed back to clindamycin as Fucidin
contraindicated late pregnancy. Now wanting to change back to Fucudin
(sodium fusidate) - is it OK BF - can't find it in Hale 2004.  Apart from
frequent effective milk removal is there anything that would help this
mother prevent problems recurring?  Could you please e-mail to me directly
