Hello, All.
       I want to second Karen's suggestion that we need to know exactly what
the "other" IBCLC actually said to the mother.  I would hope that we could
give each other the benefit of the doubt first, and talk to the (possible)
"offender" about what we have heard.  I must confess that in my younger,
much-less-wiser days, I have been guilty of saying I couldn't believe so-and-so said
such-and-such to a mother, only to have to eat crow later when I learned it wasn't
true.  I have also been on the receiving end of this, accused of saying
something (awful/inappropriate/whatever) that I absolutely never said.  I learned
to attribute this "inaccurate reporting" to the tired, overwhelmed, confused
brains of new mothers, who just don't understand what was really said to them
but then go on to complain to someone else that they were told such-and-such.
       I am thinking that, in the case of inappropriate bottle nipple usage
(as nipple shield), you could approach the IBCLC who offered this idea to the
mother and say something to the effect, "I wonder if you realize that this idea
can be quite dangerous."  And then share Linda's scary report.  I just feel
that we need to work with each other before jumping right to reporting.
However, that said, I do understand that there will be some folks who just keep on
doing dangerous things.  And, in those situations, I would appreciate continued
discussion of "where do we go from there?"
