We have a policy at our hospital titled "Ingestion of Incorrect Breastmilk"
which goes over what procedures to take when an infant is given the wrong
breastmilk.  In a nutshell, physician writes order for tests of the "donor
mother" - HIV rapid testing, hepatitis BsAg, Hepatitis C AB, HSV Igm
(infection control pays for tests).  Recipient mother should also be
encouraged to be tested - otherwise there is no baseline for proof of
transmission from breastmilk exposure.  Recipient infant should be given
prophylaxis within 2 hours of exposure if donor mother refuses HIV rapid
testing, or if results of HIV rapid testing is positive.  Hepatitis B
vaccine to be given immediately to recipient infant (unless already
vaccinated).  Follow-up testing for recipient baby @ 6 weeks, 3 months, and
6 months (HIV, Hep BsAg, Hep C AB)
Counseling is given to assure mom that risks are extremely low!
