Lynn asks:

Why should it be necessary to provide evidence for NOT doing
something?  First there should be evidence for DOING it.  What is
the "suportive literature" for the policy in the first place?

Reminds me of the story of the woman who always cut the end off a
roast before putting it in the oven.  When asked why, she said it
was because "we always do it that way".  She asked her mom why and
her mom said because her mom always did it that way.  On asking
Grandma why the end of the roast should be cut off, Grandma said,
"Oh, I always did that because the pan I used was too short for the
roast."
I wonder how many current practices based on "we've always done it
that way".  How many long standing policies started out because of a
"short pan"?

All P&Ps at our hospital have to be reviewed every so many years.
(I think it may be a JCAHO requirement since there was always a rush
to get them current before every JCAHO visit!)  There has to be
current references for each one as part of the review/revision.
This should at least make people think about whether current
practice is evidence-based.
