Hi Louise,

My first posting to Lacnet, and am experiencing a steep learning curve
myself, so may flub bits and pieces..."many" of the babies have lactose
intolerance and must d/c nursing? That seems a bit odd, though I know it
can occur. Questions to ask: How is one arriving at that diagnosis? Does
a family history of lactose intolerance exist? What is mom's dairy intake
like? Can she eliminate dairy from her diet with a resulting decrease or
elimination of baby's symptoms? Is it possible you might be seeing
"hyperlactation syndrome" (a colic characterized by gassiness, emesis,
frequent stooling, that doesn't go away when allergens are removed from
mom's diet), or something akin to it? A number of the moms I see have
tried offering only one breast per feeding, making sure to get hindmilk,
thereby decreasing the amount of lactose that ends up in baby's bowel.

That was not an original thought, of course, but one my patients took from
"Nursing Mother's Companion" by Kathleen Huggins. PubMed has a number of
abstracts about lactose intolerance and breastfeeding...are you able to
access those? I'm sure my more learned colleagues have many more
suggestions....just thought I'd try to help. And thank you to everyone who
posts--I've learned SO much! ~Linda M. Duce RN, CNM, Lactation Program
