I haven't noticed mention in this discussion of simply leaving baby uninterrupted, skin-to-skin with mom following birth, until that first latch and feeding are completed.  Unnecessary (routine) intervention in these critical moments likely undermines most breastfeeding pairs.  Even if it is inadvertent, it could be easily rectified...

Maybe a 'First do no harm' could be a good opener...
